Why Does the Patient Need A Summary Care Record? (SCR)
Storing your coded information in one place makes it easier for healthcare staff to treat you in an emergency, or when your GP practice is closed.
This information which could make a difference to how healthcare staff decide to care for you, for example which medicines they choose to prescribe for you.
Only healthcare staff involved in your care can see your Summary Care Record. Patients can also now view their own SCR via our online services facility. Should you wish to view your Summary Care Record please contact our Practice Manager to request access.
Patient Access requires two pieces of identification one being photo id the other with current address, please note access will not be granted without these two pieces of identification which have to be verified by the Practice Manager and Lead GP.
It will take the Practice up to 40 Days (as applicable to Subject Access Request and in accordance with the Data Protection Act 1998) to review your application and grant access if appropriate.
